FLATBED CONTRACTOR - MIDWEST REGIONAL BENEFITS AND QUALIFICATIONS

COMPENSATION SCHEDULE
COMPENSATION: 75% on 100% of revenue (Must have 1 year Flatbed exp. within past 3 years)
FUEL SURCHARGE: We pass on 100% of Fuel Surcharge billed to customer.
HOME TIME: Home Weekly. Dispatchers work with operators to get them home when they want. NO FORCED DISPATCH.
SAFETY BONUS: $50.00
is paid quarterly for attending a safety meeting.
REFERRAL BONUS: Any experienced driver or contractor referred by you that completes 30 days service entitles referring driver to a $800.00 referral bonus.
ACCESSORIAL PAY
TARP PAY: We pay 100% of contracted tarp pay.
SCALE RECEIPTS: Authorized scale receipts reimbursed.
DETENTION PAY or TRUCK ORDERED NOT USED: We pay 100% of what we bill and collect from customer.
PLATES, PERMITS, ESCROW, AND INSURANCE REQUIREMENTS
FREE BASE PLATES: A.T.S. base plates are provided for contractors at no cost. Contractors choosing to run their own base plates will be reimbursed monthly based on the cost per state and any limits set. Credit will be given toward the base plate reimbursement on the second week of the month following the month being credited subject to your lease remaining in an active status.
FREE PERMITS: Company provided at no cost to the contractor.
ESCROW: Mandatory $1,000 escrow account deducted @ $100.00 weekly for first 10 (ten) weeks. Additional trucks require a $500.00 escrow..
PHYSICAL DAMAGE: Industry Low 2.75% (annual) of tractors stated value; paid weekly.
NON TRUCKING INSURANCE: $28.00 per month
CARGO, PUBLIC LIABILITY: No cost.
OCC/ACC: Required of all contractors. $128.00 per month.
QUALCOMM RENTAL: $7.00 per week usage and insurance. (NO installation charge. NO charge for unit.)
FUEL TAXES: Filed quarterly, deficits are deducted through settlements in $150 increments at the end of quarter.
EQUIPMENT & DOCUMENT REQUIREMENTS TO LEASE ON
- Three axle, sleeper cabs, diesel powered tractors.
- Maximum tractor dry weight is 18,500 lbs.
- Maximum trailer weight is 11,000 lbs
- 48’ x 102” flat bed trailer. Prefer spread axle, air ride but not required.
If trailer does not have bulk head your tractor must have headache rack (available through Arnold, see general information below.)
- Tractor and Trailer must pass Federal D.O.T. and Company inspections.

| TRACTOR ALTERATIONS |
| As the lessee of the tractor we know that you will take pride in its appearance. Adding chrome, having your name painted on the tractor, adding a few lights or changing the drivers seat are items that enhance the value, comfort, and appearance of your tractor. However, as the titleholder, we require that any major alterations to the tractor be pre-approved by the maintenance department. This would include items dealing with the drive train, tractor color, murals, etc. |
| MAINTENANCE ESCROW DETAILS* |
- Settlement deducted into an interest bearing Maintenance Escrow Account for the entire term of the lease.
- Escrow is established exclusively for the performance of maintenance and repairs as contractually outlined.
- Money in the escrow fund may be withdrawn by Contractor at any time, for use only in the required maintenance and repair of leased vehicle through Road Breakdown or repairs performed in any ATS maintenance facility.
- Contractor is solely responsible for the Maintenance of the Leased Tractor.
- ATS Road Breakdown will administer the Escrow Account through the driver, providing proper receipts and documentation for parts/repairs.
- Escrow balance cannot be in the arrears. All deficits will be addressed via the Addendum G process at time they occur.
- ATS will provide basic service and repairs at its discretion and shop availability, except for major motor repairs such as, but not limited to A/C, clutch, transmission, major motor, etc.
- PM schedules 36,000 miles between ATS services, 24,000 maximum for all outside vender service.
- Driver required to bring in unit to an ATS shop quarterly for FHWA inspections.
- Contractor must complete and turn in to Log Department, all monthly maintenance records (DOT Requirement)

The Contractor has the following options regarding the Maintenance Account at the end of the contract.
• Continue weekly settlement deductions
• Discontinue weekly settlement deductions
• Take all or a portion of the money out of escrow
• Close the account
NOTE: A contractor leasing a tractor through Arnold Transportation’s Lease Purchase Program is required to be the sole driver of the tractor purchased until paid in full. |
GENERAL INFORMATION
HEADACHE RACK: Headache rack available for trucks not equipped with one, $700.00 mounted. Financing not available.
SECUREMENT EQUIPMENT: Can be purchased from ATS. Complete set is as much as $2,213.74. A down payment of $300 is required and the remainder will be set up in payments of $100 per week Can buy just what you need.
PASSENGER PROGRAM: Eligible riders include spouse, your children eleven years and older. Other immediate family members include parents, grandchildren, brothers, sisters, aunts, uncles and significant others. etc.
NOTE: There is a 90-day waiting period except for drivers with over 2 years driving experience and good driving record. Contractors-pets allowed.
OPERATIONAL AREA: Primary running areas are Florida, Georgia, Alabama, Tennessee, North and South Carolina, Mississippi, and Louisiana. There are occasions when you will run outside this region.
TERMINAL LOCATIONS: Main terminals located in Jacksonville, FL., Camp Hill, PA., Grand Prairie, TX., Baldwinsville, NY., with satellite terminals in Austell, GA., Dayton, OH., Houston & Paris, TX., Muskogee, OK.
DRIVING RECORD
Ask your Recruiter for details. All drivers must have a Class “A” C.D.L. and 1 Year Flatbed Experience within the past 3 years.
